白内障手术切口后方的皮质处理方法 被引量:3

摘要 目的探讨手法小切口白内障手术中清除切口后方的皮质方法。方法白内障连续病例240例(240眼)随机分为4组施行不同方式吸出切口后方的皮质。第1组采用J形注吸针头在切口后方进行直接皮质吸出:第2组用J形针头通过侧切口进入前房进行切口后方位的皮质吸出;第3组取用U形注吸针头在切口后方位施行皮质吸出;第4组人工晶状体植入囊袋内并旋转,使切口后方的残留皮质游离,然后直接吸出。结果术中及术后综合分析4组不同的吸出切口后方位皮质方法,从时间、清除率、并发症等方面来观察,各有利弊,故第3组方法采用U形注吸针头吸出切口后方的皮质优于其他3组方法。结论手法小切口白内障手术中应用U形注吸针头吸出切口后方的残留皮质的处理方法在临床应用中有其优点。 Objective To investigate a simple,fast and safe way for the mangelpent of cortex behind the small incision during cataract surgery. Methods We randomly divided 24 patients (24 eyes) involved in cataract into 4 groups and used different ways for cortex extraction. Group 1 : Cortex extraction was practiced by "J" type irrigating/aspirating cannula directly through the incision.Group 2: Cortex extraction was practiced in anterior chamber through side incision by "J" type irrigating/aspirating eannula. Group 3 : Cortex extraction was practiced by "U" type irrigating/aspirating cannula directly through the incision. Group 4:We let free the residual cortex by implantation and rotation of intraocular lens, then made cortex extracted directly. Results Considered by time, clearence rate and complications during the operation and postoperatively, we found that each group had its own advantages and disadvantages. Group 3 is better than the other three groups. Conclusion Using U irrigating/aspirating eannula for cortex extraction behind incision position during small incision cataract surgery can be used widely in clinical work.
