摘要
目的探讨重型再生障碍性贫血(SAA)患儿外周血CD4+CD25highFoxp3+Treg细胞与CD5+B细胞的变化及其相关性。方法收集36例SAA患儿(SAA组)及60例健康体检儿童(健康对照组),采用流式细胞术检测二组外周血CD4+CD25highFoxp3+Treg细胞与CD5+B细胞水平,并进行比较,同时分析SAA组此2种细胞数量的相关性;将免疫抑制治疗后有效的SAA患者划为治疗有效组,检测其CD4+CD25highFoxp3+Treg细胞与CD5+B细胞水平,并与治疗前进行比较,了解其变化趋势。结果SAA患儿外周血CD4+CD25highFoxp3+Treg细胞比例[(1.56±0.77)%]显著低于健康对照组[(3.73±0.93)%](P<0.01);CD5+B细胞比例在SAA组为(21.47±6.89)%,显著高于健康对照组[(12.21±6.18)%](P<0.01);SAA组CD4+CD25highFoxp3+Treg细胞比例与CD5+B细胞比例呈显著负相关(P<0.01)。治疗有效组CD4+CD25highFoxp3+Treg细胞比例为(2.62±0.75)%,较治疗前明显回升(P<0.05),但仍较健康对照组低(P<0.05);CD5+B细胞比例为(11.75±8.12)%,较治疗前显著下降(P<0.01)。结论CD4+CD25highFoxp3+Treg细胞与CD5+B细胞的比例改变可能参与儿童SAA的发病,CD4+CD25highFoxp3+Treg细胞的比例下降可能与CD5+B细胞的增高有关。
Objective To investigate the quantitative changes of CD4^+ CD25 ^highFoxp3 ^+ Treg lymphoeytes and CD5 ^+ B lymphocytes in peripheral blood of children with severe aplastic anemia ( SAA ) and analyze the relationship between them. Methods Quantities of CD4^+ CD25^high Foxp3^+ Treg lymphocytes and CD5^+ B lymphocytes in peripheral blood of 36 children with SAA ( SAA group) and 60 healthy children (healthy group) were assayed by flow cytometry. The correlation between the quantitative changes of CD4^+ CD25 ^high Foxp3^ + Treg lymphocytes and CD5^+ B lymphocytes was analyzed. Results The quantity of CD4^ + CD25^ high Foxp3^ + Treg lymphocytes was significantly lower in SAA group [ (1.56 ± 0. 77 )% ] than that in healthy group [ (3.73 ± 0. 93 )% ] (P 〈 0.01 ), while the quantity of CD5 ^+B lymphocytes in SAA group was significantly higher than that in healthy group [ ( 21.47 ± 6.89 ) % vs ( 12.21 -+ 6.18 ) %, P 〈 O. O1 ], The quantity of C D4 ~ CD25 h^gh Foxp3 ~ Treg lymphocytes showed negative correlation with CD5 +B lymphocytes( P 〈 O. 01 ). After effective immuno suppressive therapy(IST) treat- ment, the quantity of CIM ~ CD2s ~gh Foxp3 + Treg lymphocytes [ ( 2.62 + O. 75 ) % ] ascended significantly compared with pretherapy ( P 〈 O. 05 ) , but descended significantly compared with healthy group ( P 〈 O. 05 ). CD5 * B lymphocytes [ ( 11.75 + 8.12 ) % ] descended significantly compared with pretherapy ( P 〈 O. 01 ). Conclusions CD4 ~ CD25 high Foxp3 + Treg lymphocytes in peripheral blood of children with SAA signifi- cantly decreased and CD5 *B lymphocytes increased,which may be related with the onset of the disease. The quantitative decrease of CD4^+ CD25^high Foxp3^+ Treg lymphocytes may be related with the increasing of CD5^ +B lymphocytes.
