摘要
目的:探索全面部骨折手术中既不影响面中部骨折复位又不妨碍术中咬合关系对位,且避免气管切开的麻醉插管新方法。方法:在10例全面部骨折手术中,先行常规的经口气管插管,待全麻稳定后,制备口底至颌下区的隧道,然后将气管导管由口内引导至颌下区再接麻醉机全麻,手术后再将气管导管送回口腔,缝合口底和颌下切口。结果:所有病例插管经过顺利,麻醉满意,且不影响手术操作,术后无口底出血、感染、气道阻塞,神经损伤等并发症。结论:口底-颌下入路气管插管是一种适合于全面部骨折的安全、微创的麻醉新方法。
Objective： To explore a new way for intubation in operation of panfacial fracture, without disturbing the reduction of midface fracture and occlusion, avoiding tracheotomy. Methods： A total of 10 patients were routinely intubated through oral cavity,then a tunnel was prepared from mouth floor to submandibular region.The tube was guided to submandibular region through the tunnel and connected to the anesthesia machine and then pulled back to oral cavity after operation with the incisions were sutured. Results：All of the patients got satisfactory general anesthesia and, without the complications of hemorrage,infection,obstruction of airway and injure of nerve, the reduction of maxillary fracture was not disturbed. Conclusion：The intubation through mouth floor-submandibular tunnel is a safe and minimally invasive method that is adaptable for panfacial fracture.
