盐酸托烷司琼联合地塞米松预防肺癌化疗急性呕吐疗效观察

Clinical efficacy of tropisetron hydrochloride and dexamethasone in relieving emesis induced by chemiotherapy in lung cancer patients

目的评价盐酸托烷司琼联合地塞米松对肺癌化疗引起的急性呕吐反应的预防作用。方法75例含顺铂化疗的肺癌患者采用自身前后对照方法观察盐酸托烷司琼单用与联合应用地塞米松对化疗致急性呕吐反应的预防作用。40例肺癌患者化疗第1周期单用盐酸托烷司琼,第2周期联合地塞米松;35例肺癌患者化疗第1周期用盐酸托烷司琼联合地塞米松,第2周期单用盐酸托烷司琼。按WHO标准评价化疗24h内患者恶心呕吐反应及临床副作用。结果盐酸托烷司琼单用的完全控制率为73.3%(55/75例),有效率为86.7%(65/75例);两药联合应用的完全控制率为88%(66/75例),有效率为96%(72/75例),两药联用时的完全控制率和有效率均明显高于单药应用时,χ2值分别为5.17和4.13,P均<0.05。单药和两药联用时副作用均轻微,无显著性差异(χ2=0.77,P>0.05)。结论盐酸托烷司琼与地塞米松联合应用可以明显减轻肺癌患者化疗的急性呕吐反应,副作用无明显增加。

[Objective ] To evaluate the clinical efficacy of tropisetron hydroehloride and dexamethasone in relieving emesis induced by ehemiotherapy in lung cancer patients. [Methods] 75 lung cancer patients with chemiotherapy were designed with serf-across comparison methods. 40 cases of patients were administrated with tropisetron hydroehloride alone for the first course, with combination of dexamethasone for the second course. The other 35 eases were firstly administered with the above therapeutic alliance,and secondly administrated with tropisetron hydrochloride alone. The clinical eficacy of relieving emesis and the side effeet of the therapy were observed. [Results ] Complete eontrol rate of tropisetron hydroehloride alone and the therapeutic alliance was 73.3% （55/75） and 88% （66/75） respectively. The complete control rate of the therapeutic alliance was greatly higher than that of tropisetron hydroehloride alone （X2=5.17, P 〈0.05）. Effective rate of the therapeutie alliance 96% （72/75） was significandy higher than that of the tropisetron hydrochloride alone 86.7% （65/75） （X2=4.13, P 〈0.05）. The side effeet of the therapeutic alliance was similar to that of the tropisetron hydrochloride alone （X2=0.77,P 〉0.05）. [Conclusion] The therapeutic alliance of tropisetron hydroehloride and dexamethasone could increase the effieaey in relieving emesis induced by ehemiotherapy in lung cancer patients compared with the tropisetron hydroehloride alone, and there is no obvious side effeets on the therapeutie alliance.
