摘要
目的观察缺血性脑梗死患者血浆内皮素/降钙素相关基因肽含量变化与脑血流变化之间的关系。方法40例诊断为缺血性脑梗死患者和40例志愿者分成2组:脑梗死组给予常规标准化治疗,定时(入院6h内、24h、20d)给予床旁经颅多普勒监测脑血流状况并同时采集静脉血,用放射免疫法测定血浆中ET、CGRP浓度水平。对照组40名健康体检志愿者进行1次经颅多普勒检查并同时采集静脉血用放射免疫法测ET、CGRP,并进行统计。结果脑梗死组和对照组的血ET、CGRP分别为:(57.31±11.14)ng/L,(23.64±6.38)ng/L和(36.45±8.64)ng/L,(19.93±4.32)n。2组ET、CGRP、有明显差异,差异有显著性(P<0.01);经颅多普勒监测脑血流状况脑梗死组血管异常率显著高于正常对照组(P<0.01)。结论ET和CGRP的活性增强,将预示着有脑血管异常的发生,可能存在脑血管痉挛或梗塞所造成的脑缺血缺氧。
[Objective ] To observe the clinical study of plasma endothelin/calcitonin gene related peptide and dynamic changes of rCBF in patients with cerebral infarction. [Methods ] 40 patients suffered from cerebral infarction and 40 volunters were divided randomly into two groups: cerebral infarction group was deal with common standard method treatment, the conditions of cerebral blood flow was observed with transcranial doppler ultrasound according to the time after admission (6h, 24h, 20d) and serum ET, CGRP were measured at the same time and 40 volunters the conditions of cerebral blood flow was observed with transcranial doppler ultrasound and serum ET, CGRP were measured at the same time, the data were analyzed statistically. [Results ] The serum ET, CGRP in cerebral infarction group and volunteers group were (57.31±11.14) ng/L, (23.64±6.38) ng/L and (36.45±8.64) ng/L, (19.93±4.32) ng/L respectively. Between the two groups the serum ET and CGRP were different significantly (P 〈0.01). Under the observation of transcranial doppler ultrasound ,the oecurrenee of abnormal changes in velocity of cerebral blood flow showed more common in cerebral infarction group than in volunteers group (P 〈0.01). [ Conclusions ] The enhance- ment of serum ET and CGRP activation will indicate the possibility of cerebral artery abnormality and cerebra hypoxie-ischemic which caused by eer.ebral infarction and eerebral vasospasm.
