Significance of Community Based Health Management in Prevention of Chronic Diseases

为迎接慢性病防治的挑战,我国进行了新一轮的社区卫生服务改革.但由于社区卫生工作存在的诸多问题,使慢性病防治工作依然形势严峻.健康管理作为新的卫生服务理念,对于慢性病防治和社区卫生服务都有重要意义.健康管理有利于调整社区卫生服务模式;有利于慢性病的双向转诊服务;有利于对社区慢性兵重点人群监控,因此在社区慢性并防控方面有重大意义.

Our government has taken a new reform in community health service to meet the challenges in prevention of chronic deseases. However, prevention of chronic deseases is still a hard task because of many problems in community health service work. As the innovative method of health service, health management plays an important role in chronic disease prevention and community based health service. Health management contributes to adjusting community health service model, two-way referral system in prevention of chronic deseases, and monitoring the key patients with chronic deseases in community, therefore it plays an important role in prevention of chronic deseases in community.
