经口气管插管患者口腔护理方法的Meta分析

Meta-analysis Applied in Oral Care for Patients with Orotracheal Intubation

目的评价冲洗法与传统法对经口气管插管患者口腔护理的效果。方法通过数据库检索国内相关文献,并按Cochrane协作网提供的Meta分析对其进行定量综合分析,比较两种不同口腔护理方法的护理效果。结果共11篇文献1028例患者符合纳入标准。冲洗组口腔异味发生率明显低于传统组,合并OR值为0.04,95%CI为(0.01,0.10);舌苔转薄率存在异质性(χ2=7.87,P<0.05),采用随机效应模型分析,合并RR值为3.20,95%CI为(1.66,6.15);口腔感染及肺部感染发生率均不存在异质性,采用固定效应模型分析,表明冲洗法口腔护理明显降低了口腔感染和肺部感染的发生率,与传统法比较差异有统计学意义(P<0.05)。结论冲洗法口腔清洁效果明显优于传统法。

Objective To evaluate the effect of oral care of traditional method and washing method for patients with orotracheal intubation. Methods The relative domestic literatures were retrieved from the database. Meta - analysis provided by Cochrane operation network was used for quantitative aggregate analysis for related articles. The effects of oral care were compared between the washing method and the traditional method. Results One thousand and twenty - eight patients from eleven literatures met to internalizing standard. The incidence rate of halitosis of the washing group was obviously lower than that of the traditional group, the pooled OR value was 0.04 （95% CI： 0. 01,0. 10） ; the rate of thinning transformation of tongue coating was of heterogeneity （Х^2 =7.87, P 〈0. 05） by analysis with random effects model, the pooled RR value was 3.20 （95% CI： 1.66, 6. 15 ） ; the incidence rates of oral infection and lung infection had no heterogeneity by analysis with fixed effects model, which illustrated that, compared with traditional method, the oral care of the washing method obviously reduced the incidence rate of oral infection and lung infection （ P 〈 0. 05 ）. Conclusion The cleaning effect of the oral care of the washing method is obviously superior to the traditional method.
