磁共振对比剂对脑肿瘤单体素氢质子波谱的影响 被引量:2

摘要 目的探讨磁共振对比剂Gd-DTPA对氢质子磁共振波谱的影响,进一步提高脑肿瘤的诊断水平。方法选取29例脑肿瘤患者(胶质瘤11例,脑膜瘤18例),先进行常规MRI扫描,包括三平面定位,横断面T1WI、T1WI、FSE、FLAIR扫描和矢状面扫描。于T1FLAIR序列上选取肿瘤最大层面设置感兴趣区进行单体素氢质子波谱扫描(1H-MRS)。经肘静脉团注磁共振对比剂Gd-DTPA后行增强扫描,于肿瘤显示增强10min后再次于同一区域行1H-MRS,采用GE Functool软件分析图像;比较注射对比剂前后肿瘤组织主要代谢产物NAA、Cr、Cho各峰值、峰下面积和最大半高宽值。结果所有病例注入对比剂前后各代谢产物峰值、峰下面积、和最大半高宽均无明显变化(P>0.05)。结论Gd-DTPA不会对波谱图像产生影响,可提高1H-MRS诊断的准确性;1H-MRS宜在增强扫描后进行,与MRI增强扫描联合应用有利于脑肿瘤的诊断和鉴别诊断。
