新生儿缺氧缺血性脑病血清抵抗素与高敏C反应蛋白含量变化的临床研究

Dynamic changes of serum resistin, high-sensitive C-reactive protein （hs-CRP） and hypoxic-ischemic encepha lopathy

目的探讨新生儿缺氧缺血性脑病（HIE）患儿血清抵抗素和高敏C反应蛋白（hs-CRP）水平的动态变化。方法分别测定116例HIE患儿及40例正常新生儿血抵抗素与hs—CRP水平，并进行观察与比较。结果生后2h内HIE各组（重、中、轻）抵抗素与hs．CRP水平明显高于对照组，差异有统计学意义（t=18．45，16．72，13．17与t=19．78，17．31，12．58，均P〈0．01），并且随着病情程度的加重而逐渐升高。抵抗素与hs—CRP水平生后第1天时最高，轻度组第3天基本恢复正常；中度组第3天明显下降，第10天基本恢复正常；重度组第3天无明显下降，第10天有明显降低，但仍高于对照组（t=11．27与t=11．36，均P〈0．01）。HIE患儿血清抵抗素水平与hs-CRP水平呈显著正相关（r=0．62，P〈0．01）。结论新生儿HIE血抵抗素与hs—CRP水平变化与HIE严重程度有关。

Objective To evaluate the dynamic changes of serum resistin and high- sensitive C-reactive protein（hs-CRP） levels in neonates with hypoxic-ischemic encephalopathy （HIE）. Methods Serum resistin and hs-CRP levels were measured in 116 neonates with HIE （ 18 cases of HIE were mild; 17 moderate; and 11, severe） and in 40 normal term neonates by enzyme-linked immunosorbentassay and rate nephelometry immunoassay on admission, 1st day, 7th day and 10th day after birth. Results Serum resistin and hs-CRP levels in infants with HIE were significantly higher than that in control in 2 hours after birth（ t = 18.45, 16.72, 13.17 ; 19.78, 17.31, 12.58, all P 〈 0.01 ）. Serum resistin and hs-CRP levels in infants with mild HIE showed no significant differences from the control on third day. Moderate HIE peaked on the first day after birth, then decreased gradually and resumed on the third day after birth. Severe HIE peaked on the first day after birth and then lasted until the third day, while Serum resistin and hs-CRP levels significantly decreased on the tenth day after birth, but was still significantly higer than that in control （t = 11.27, 11.36, all P 〈 0.01 ）. Serum resistin level significantly related to Serum hs-CRP levels （r = 0.62, P 〈 0.01 ）. Conclusion The present data suggests that the changes of Serum resistin and hs-CRP levels are correlated with severity of HIE.
