三维适形放疗治疗脑转移瘤的对照研究

Control study of 3D-CRT for brain metastastic encephaloma

目的评价脑转移瘤放疗的合理方法及其效果。方法330例脑转移瘤患者平均分为两组,分别行全脑照射加三维适形推量放疗(推量组)和单纯三维适形放疗(适形组):推量组患者行全脑照射,肿瘤量达30Gy/10次后行三维适形放疗推量,累积剂量达70 Gy。适形组仅行三维适形放疗,生物有效剂量与前者一致。结果所有患者在治疗后随访一年,推量组局控率为73%,适形组局控率为62%,差异有显著性(P<0.05)。推量组一年生存率为41%,适形组一年生存率为39%,差异无显著性(P>0.05)。结论两组一年生存率相似,推量组局控率高。

Objective o investigate the effect of 3D - CRT for brain metastases. Methods One hundred and sixty -five patients treated by 3D - CRT with whole cerebral radiotherapy （WDRT ＋ 3D - CRT） were retrospectively compared with 165 patients treated by 3D - CRT only. Results The patients treated by WBRT ＋ 3D - CRT had significant benefit in local control rate （73% vs 62% ）, respectively （P 〈0.05） . But there was no difference in terms of one - year survival rate between the two groups （41% vs 39% ）, respectively （ P 〉 0.05 ）. Conclusions The patients treated by WBRT ＋ 3D - CRT had more benefit in local control rate. But there was no difference in terms of one -year survival rate between the two groups （ P 〉 0.05 ）.
