联合DWI,FLAIR和MR常规序列在急性脑弥漫性轴索损伤中的诊断价值 被引量:4

摘要 目的探讨联合应用扩散加权成像(diffusion-weighted imaging,DWT)、液体衰减翻转恢复(FLAIR)序列和常规MRI序列在急性脑弥漫性轴索损伤(diffuse axonal injury,DAI)中的诊断价值。方法回顾分析27例经临床诊断的急性DAI患者的MR影像资料,所有患者均行T_1WI,T_2WI,FLAIR及DWI序列扫描,分析其信号特征,比较各序列脑内病灶的显示率。结果 DWI序列显示病灶最多、最敏感;FLAIR只有部分病灶显示;常规T_2WI,T_1WI较FLAIR敏感性差,其中,T_1WI显示率最低。结论对急性DAI病灶检出率DWI序列较FLAIR及常规T_1WI,T_2WI高,可为急性DAI的诊断治疗提供科学客观的影像学依据,对临床疑有DAI患者,DWI应做为MR检查的常规序列。 Objective To investigate the value of DWI, FLAIR and conventional MRI sequences for diagnosis of acute DAI ( diffuse axonal injury). Methods The MRI data of 27 cases with acute DAI diagnosed by clinic were analyzed retrospectively. All the patients underwent MRI examination with FLAIR and DWI sequence. The signal intensity features and detection rate of lesions were compared in all sequences. Results DWI sequence detected more lesions than that in other two sequences. FLAIR did not detect all lesions. SE sequences had lower sensitivity than that in the FLAIR. SET1WI sequence had the lowest sensitivity. Conclusion DWI and FLAIR have higher sensi- tivity than that in SE sequence,and can provide more image information for the diagnosis and treatment of acute DAI. DWI should be used as the routine sequence in the MRI examination for the patients who probably have acute DAI.
