手法矫正寰枢关节骨错缝的有效性研究

Effectiveness of Manual Reduction in Correcting Articulatio Atlantoepistrophica Semidislocation

目的观察手法矫正颈椎病寰枢关节骨错缝的有效性。方法对发生寰枢关节骨错缝的31例颈椎病患者采用手法矫正治疗,观察治疗前后症状、体征和X线检查的变化情况。结果手法矫正寰枢关节骨错缝的愈显率为96.8%(30/31),总有效率为100%。手法矫正前齿状突偏歪100%(31/31),经过手法矫正后降低至61.3%(19/31),其中完全矫正率为38.7%(12/31);治疗前后寰齿侧间隙比值差异有统计学意义(P<0.01)。手法矫正前寰枢关节间隙不对称者占70.9%(22/31),手法矫正后降低至51.6%(16/31)(P<0.01),其中完全矫正率为19.3%(6/31);治疗前后寰枢关节间隙比值差异有统计学意义(P<0.05,P<0.01)。结论寰枢关节矫正手法可有效矫正颈椎病寰枢关节骨错缝。

Objective To observe the effectiveness of manual reduction for articulatio atlantoepistrophica semidislocation. Methods Thirty-one cases of articulatio atlantoepistrophica semidislocation were treated by manual reduction. The clinical effectiveness was confirmed by symptoms, signs, and X-ray. Results The cure and marked effective rate was 96.7% and the total effective rate was 100% ; the un-mediacy odontoid deviation accounted for 100% before treatment and 61.3% after treatment（P 〈0.01 ） ; asymmetric articulatio atlantoepistrophica accounted for 70.9% before treatment and 51.6% after treatments（P 〈0.01 ） ; there were significant differences in the space of articulatio atlantoepistrophica between pretreatment and posttreatment（ P 〈0.05, P 〈0.01 ）. Conclusion Manual reduction can effectively correct articulatio atlantoepistrophica semidislocation.
