摘要
目的:探讨超声检查对羊水污染的预测价值。方法:用日本产ALOKASSD-500型超声仪对126例足月妊娠临产者的产前羊水、产时羊水及破膜时宫内残余羊水进行超声检测,并与羊水实际污染情况相对比。结果:超声检查对羊水污染的敏感性、特异性、阳性预测值、阴性预测值分别为:92.59%、26.26%、25.51%、92.85%,对羊水Ⅱ~Ⅲ度污染的阳性预测值为100%。结论:超声提示羊水混浊,主要反映羊水的物理性状,对正常羊水中的胎脂等有形成分与污染时的粪块难以鉴别,故超声难以诊断羊水污染,但若超声提示羊水无混浊时。
Objective:To study the predictive value of ultrasonography for meconium staining amniotic fluid (MSAE).Methods:126 cases of term pregnant women were examinated by ultrasonography for amniotic fluid(AF)before labor or remain and out of the uterus when the membrane ruptured.Results:The ultrasonic images of AF were compared with real AF.The results showed that the sensitivity,the specificity,the positive and negative predictive values of ultrasonography for MSAF were 96 15%,27 00%,25 51%and 92 85%,respectively,and the positive predictive rate for Ⅱ~Ⅲ degree of MSAF was 100%.Conclusions: The ultrasound examination only reflect the physical charater of AF when it showed turbid.Since we can't distinguish some normal ingredient such as the fetal fat in AF from meconium particles,it's difficult to diagnose MSAF by ultrasonography only.There was few possibility for MSAF if ultrasound examination did not show turbid AF.
