摘要
目的:探讨超声诊断下肢深静脉血栓的临床价值。方法:应用HP8500多功能彩超仪,7.5MHz线阵探头,外周血管软件检查临床疑下肢深静脉血栓32例,另取12例正常人24条下肢深静脉作对照。2DUS显示形态学改变,CVI及PW模式观察血流状态。结果:超声检出下肢深静脉血栓形成25例,除外血栓形成7例。2DUS显示血栓形成部位血管内径明显增大,与患者健侧同水平深静脉内径进行配对t检验,t=13.82,P<0.0001,有显著性差异,管腔内见实质回声,加压探头管腔不易压扁,有血栓定位作用;CUI及PW模式进一步证实并可检则血栓阻塞程度。结论:无创性超声检查下肢深静脉血栓,既可观察血管形态结构改变,又可实时显示血流状态,是诊断该病及观察疗效的可靠方法。
Objective:The application value of ultrasounograph to diagnosis lower extremity deep venous thrombosis was explored.Methods:32 patients suspected lower extremity deep venous thrombosis and 12 normal persons were examined by using the color ultrasound imaging device, 7.5 MHz linear array transducers (USA,Hp 8500). Reselts:25 deep venous thrombosis were diagnosed in the patients group.2DUS:the diameter of vein was clearly increased in deep vein thrombosis,there was significantly difference between pathologic vein and normal one (matched t test,t =13 82 P<0 000 1).Intraluminal parenchymatous echoes were seen and none of the clot bearing veins were fully compressible,the color flow and pulsed wave Dopplor signal may be useful in the detection of eccentric thrombus and partially occlusive thrombus.Conclusion:A color ultrasound has several advantages over venography,such as realtime, noninvasive,easily repeatable,free of complication,with the ability to visualize spontaneous venous flow in color without the injection of contrast material.It is the first choice to use this method for the diagnosis of lower extremity deep venous thrombosis.
