246例糖尿病患者的并发症分析

ANALYSIS OF THE COMPLICATIONS IN 246 PATIENTS WITH DIABETES

目的：探讨糖尿病患者的并发症与血糖水平、发病年龄、病程之间的关系。方法：对１９９３～１９９６年间在我院住院的糖尿病２４６例资料进行回顾性统计分析。按照ＷＨＯ１９８５年制定的标准进行诊断，糖尿病Ⅰ型４４例，Ⅱ型２０２例。按发病年龄分为青少年组、中年组和老年组。对并发症与发病年龄、病程长短和血糖水平作了分析。结果：糖尿病并发症糖尿病肾病、视网膜病变在病程＞５年后为７８２％和８８５％，＞１０年后达８２８％和１００％。肢端坏疽在病程＞１０年后达２８６％。糖尿病各种慢性并发症在老年组发生率高，急性并发症酮症酸中毒青少年组发生率高。结论：病程不同，糖尿病的并发症及感染发生的情况不同，有随病程延长而增加的趋势。发病年龄越大。

Objective: To investigate the relationship between the complications of patients with diabetes and the level of blood sugar, age of onset, and the course of the disease. Methods: Two hundred and forty-six diabetes subjects hospitalized in our hospital from 1993 to 1996 underwent retrospective statistic analysis. The patients were divided into juvenile group, middle-aged group and senile group according to the age of onset. All the patients were diagnosed with the criterion issued by World Health Organization, diabetes type Ⅰ 44 cases and diabetes type Ⅱ 202 cases. The relationship of complications of the disease to the age of onset, the course of the disease and the level of blood sugar were studied. Results: The incidence of diabetic renopathy and diabetic retinopathy were 78.2% and 88.5% respectively in the patients suffering the disease more than 5 years, and 82.8% and 100% in the patients suffering the disease over 10 years. The incidence of diabetic gangrene was 28.6% in the patients suffering the disease over 10 years. The incidence of chronic complications of diabetes was higher in senile group and the incidence of acute complications of the disease, diabetic ketoacidosis, was higher in juvenile group. Conclusion: The development of the complications and infections of diabetes vary with the disease course, the incidence of the complication is increasing with the length of the disease course. The incidence of each chronic complication of the disease is higher, as the onset age is advancing
