脑梗塞患者血浆内皮素水平的研究

Changes of endothelin in plasma in patients with cerebral infarction

目的进一步探讨内皮素（ＥＴ）与脑梗塞（ＣＩ）的发生、发展的关系。方法采用放免分析法动态观测了９５例ＣＩ患者血浆ＥＴ含量变化。结果（１）ＣＩ患者血浆ＥＴ水平明显高于对照组（Ｐ＜０．００１）。（２）ＣＩ患者血浆ＥＴ水平与病灶大小、病情轻重及是否合并其他器官损害有关；与年龄、病灶部位及病灶数目无关。（３）ＣＩ患者血浆ＥＴ水平动态变化规律为：血浆ＥＴ水平在发病后２４ｈ内开始升高，４～７天达高峰，半月后明显下降，１月后降至较正常水平稍高，与ＣＩ后脑水肿及缺血半暗带的形成时程一致。结论ＥＴ在ＣＩ的发生、发展及预后中起重要的作用。

Objective To explore the role of endothelin(ET) in the pathogenesis of cerebral infarction(CI).Methods We observed the changes of ET in plasma in 95 patients with CI by radioimmunoassay.Results 1.Compared to the control group(66.75±27.23),the levels of ET in plasma in patients with CI (112.1±48.57) was elevated significantly (P<0.001).2.The levels of ET in plasma in patients with CI was associated with the size of infarction,the severity of the disease and whether or not complicated by other organic failure,and it was not related to age,the locations of the CI and the number of the focuses.3.The trend of ET levels in plasma in patients with CI was as follows:the ET levels in plasma were began to elevate within 24 hours after the onset of CI,reach the peak on days 4～7,then became descended significantly on days 15 and became slightly higher than normal on days 30.The time course of the occurrence of brain edema and ischemia penumbra and that of the increase in plasma ET coincided precisely.Conclusion These results suggest that ET plays a key role in the pathogenesis of CI.
