摘要
目的观察高频振荡通气治疗新生儿呼吸窘迫综合征(NRDS)的疗效。方法分别用高频振荡通气(HFOV)和常频机械通气(CMV)治疗NRDS新生儿,比较治疗前后血气分析指标、机械通气时间、住院时间、治愈率情况。结果 HFOV组患儿在治疗后6 h、12 h氧浓度(FiO_2)、二氧化碳分压(PaCO_2)明显下降,氧分压(PaO_2)、PaO_2/FiO_2比值上升明显升高,与CMV对比差异有统计学性意义(P<0.05);HFOV组机械通气时间、住院时间缩短,治愈率增加,与CMV对比差异有统计学性意义(P<0.05)。结论高频振荡通气能改善新生儿呼吸窘迫综合征的肺氧合功能,减少并发症,缩短住院时间,降低病死率,较常频机械通气使用更为安全。
Objective To study the outcome of neonatal respiratory distress syndrome(NRDS) treated with high frequency oscillatory ventilation(HFOV). Methods Newborns with NRDS were treated with HFOV and conventional mechanical ventilation (CMV) respectively, and the blood gas analysis index, mechanical ventilation time, hospitalization time and curative ratio were compared before and after treatment. Results ( 1 ) Fractional inspired oxygen ( FiO2 ) and partial pressure of carbon dioxide( PaO2 ) were markedly dropped, and FiO2 , PaO2/FiO2 were markedly evelated after 6,12 hours in HFOV group. There was statistical meaning between HFOV group and CMV group ( P 〈 0.05 ) ; (2) The shorter mechanical ventilation and hospitalization time and the high curative ratio were in HFOV group. There was also statistical meaning between HFOV group and CMV group ( P 〈 0. 05 ). Conclusion The HFOV can improve the lung's oxygehation function, reduce hospitalization time and morality, and it is as safe as CMV.
