类风湿关节炎患者Th17细胞相关细胞因子的表达及临床意义 被引量:11

摘要 目的分析类风湿关节炎(rheumatoid arthritis,RA)患者外周血和关节液中T辅助细胞17(Thelp cell 17,Th17)相关细胞因子白介素-6(interleukin-6,IL-6)、白介素-17(interleukin-17,IL-17)水平,探讨其在RA中的变化特点及与实验室指标的相关性。方法用ELISA检测RA患者血清和关节液和正常人血清IL-6、IL-17水平。结果RA患者外周血血清IL-6、IL-17水平明显高于健康对照组[IL-6:(54.1±12.7)pg/mlvs(37.0±13.0)pg/ml,P<0.05;IL-17:(57.8±15.6)pg/mlvs(47.6±8.5)pg/ml,P<0.05],RA患者关节液中IL-6、IL-17水平明显高于RA外周血血清组[IL-6:(360.4±54.7)pg/mlvs(54.1±12.7)pg/ml,P<0.01;IL-17:(82.1±12.5)pg/mlvs(57.8±15.6)pg/ml,P<0.01]。IL-6水平与红细胞沉降率(erythrocyte sedimentation rate,ESR)、C-反应蛋白(C-reactive protein,CRP)、类风湿因子(rheumatoid factor,RF)呈显著正相关。结论Th17相关细胞因子在RA外周血及关节局部过度表达,提示Th17型细胞因子在RA的免疫致病机制中可能起重要作用。 Objective To investigate IL-6, IL-17 levels in blood and synovial fluid of patients with rheumatoid arthritis, their variable features in arthritis duration and relationship to laboratory markers. Methods IL-6,IL-17 levels from RA patients and controls were evaluated by enzyme-linked immunoassay (ELISA). Results Serum IL-6, IL-17 values were higher in RA patients compared with those in the controls, and so were in RA synovial fluid. There was a positive correlation between IL-16 and CRP, ESR, RF. Conclusion Th17 type cytokines overexpresses in serum and synovial fluid of RA patients, and Th17 type cytokine may play an important role in the pathogenesis of RA.
