摘要
为探索改良肺动脉/主动脉血流时间间期比定量估测肺动脉压的方法,利用多普勒超声测量主、肺动脉血流 F 值[F=PEP×PV/(AT×ET)、F_1=PEP/AT、F_2=PEP/ET、F_3=PEP/(AT×ET)、F_4=PEP×PV/AT、F_5=PEP×PV/ET],按 F_(PA)/F_(AO)法估测肺动脉压,并与心导管值对比。结果:根据 F_(3PA/3AO)估测的肺动脉收缩压、平均压与心导管值的相关性最好(r=0.86、0.81);F_(PA)/F_(AO)、F_(1PA)/F_(1AO)、F_(2PA)/F_(2AO)、F_(4PA)/F_(4AO)、F_(5PA)/F_(SAO)较差(r=0.55~0.79)。结论:在本文提出的几种改良方法中,F_(3PA)/F_(3AO)为无创定量估测肺动压的最佳方法。
In order to explore modified pulmonary to aortic blood flow time interval ratios in estimation of pulmonary arterial pressures,pulmonary and aortic F values [F=PEP X PV/(AT X ET),F1=PEP/AT,F2=PEP/ET,F3=PEP/(AT X ET),F4=PEP X PV/AT,F5=PEP X PV/ET] were calculated by Pulsed Doppler Echocardiography in 30 patients with left-to-right shunting of congenital heart disease.Pulmonary artery pressures calculated by F_(PA)/F_(AO),F_(1PA)/F_(1AO),F_(2PA)/F_(2AO),F_(3PA)/F_(3AO) F_(4PA)F_(4AO),F_(5PA)F_(5AO) were compared with pulmonary arterial pressures measured by cardiac catheterization.The results showed that estimated pulmonary systolic and mean pressures calculated by F_(3PA)F_(3AO) highly correlated with those measured by cardiac catheterization (r=0.86,0.81),and correlated better than others (r=0.55~0.79).Thus,the modified pulmonary to aortic flow time interval ratios F_(3PA)F_(3AO) is an accurate parameter for quantitative assessment of pulmonary arterial pressure.
