摘要
目的观察同种原位心脏移植术后的心电图改变。方法分析资料完整的8例同种原位心脏移植术后患者的心电图。结果观察到5种特殊的心电图表现：8例（最长1例时间达12年）均出现双重窦性P波，貌似心房分离。食管导联心电图有助于显示受体P波，供体心房起搏或房性心律失常不影响受体窦性周长。受体心房颤动合并供体房室交接区性逸搏伴逆行P波，假性心房内不完全性传导阻滞，假性完全性房室传导阻滞。结论认识这些心电图改变有助于同种心脏移植术后的心电图诊断。
Objective To observe electrocardiographic （ECG） changes after orthotopic heart transplantation. Methods ECGs from 8 patients received orthotopic heart transplantations were analyzed. Results Five specific ECG features were identified. Double sinus P waves looked like atrial dissociation appeared in all 8 cases. An esophageal lead was of benefit to revealing recipient P waves. Atrial beats or arrhythmia from donor atria did not interfere recipient sinus cycle length. Recipient atrial fibrillation coexisted with retrograde P waves from donor atrioventricular junctional escape beats. Pseudo-incomplete interatrial conduction block and pseudo-complete atrioventricular conduction block appeared. Conclusion Knowing about these ECG features is useful to make diagnosis easy in patients with heart transplantations.
