外伤性脾破裂的诊断与治疗分析

目的探讨外伤性脾破裂的诊断与治疗经验,提高救治水平。方法回顾性分析我院2001年1月～2007年12月间收治的78例外伤性脾破裂患者的临床资料。结果非手术治疗9例全部治愈;手术组69例治愈66例,死亡3例;术后未发生膈下脓肿及凶险感染(OPSI)等严重并发症。结论早期诊断、纠正失血性休克是抢救外伤性脾破裂的首要环节,手术仍是治疗脾损伤的主要方法,非手术治疗应严格掌握适应证。

Objective To explore the experience in diagnostic and therapeutic of traumatic splenic rupture. Methods The clinical data of 78 patients with traumatic splenic rupture from January, 2001 to December, 2007 were retrospectively analyzed. Results 9 patients were cured by non- operative therapy. 69 patients were cured by operative therapy, 3 patients died. no sever complication occured such as abscess under diaphragm and OPSI. Conclusion Earlier diagnostic and to correct hemorrhagic shock is the first step to rescue traumatic splenic rupture, operative therapy is still the main method in treating spleen injury, non-operative therapy should control the compliant strictly.
