老年2型糖尿病合并下肢血管病变患者血脂异常的临床观察

Dyslipidemia of the elderly T2DM with lower extremity vascular disease

目的分析老年2型糖尿病(T2DM)下肢血管病变情况包括患病率、斑块大小、内中膜增厚度,分析斑块的发生及其程度与血脂谱的关系。方法检测46例老年T2DM患者下肢血管彩超、血糖、血脂、血压、糖化血红蛋白等,根据血管病变的轻中重程度分为两组。(轻中度下肢血管病变组为A组,重度下肢血管病变组为B组)。结果两组TG、TC、HDL-C、LDL-C、ApoA1、ApoB均高于正常,其中B组各项值较A组高(P<0.05)。提示2型糖尿病合并下肢血管病变的患者血脂升高,且病变程度越重血脂越高。结论老年2型糖尿病患者血脂谱异常的程度与其下肢血管病变程度成正相关性,下肢血管动脉硬化程度更高,影响下肢血液循环,积极的调脂治疗可以预防老年2型糖尿病患者的下肢血管病变的发生和发展。

Aim To analyze the type 2 diabetes （T2DM） of the elderly, including lower extremity vascular disease prevalence rate, plaque size, and the increase in membrane thickness, and to analyze the occurrence of arteriosclerosis plaque and the relationship between its degree and lipid profile. Methods Clinical data of 46 elderly patients with T2DM were tested with color Doppler ultrasound of lower extremity blood vessels, blood glucose, blood lipid, blood pressure, HbA1C and so on. In accordance with the degree of vascular disease, it may be divided into two groups （mild and moderate lower extremity vascular disease as group A and severe lower extremity vascular disease as group B）. Results The data of TG, TC, HDL-C, LDL-C, ApoA1, ApoB of two groups were higher than normal. The value of group B is higher than group A （ P 〈 0.05 ）. Blood lipid of type 2 diabetes patients with lower extremity vascular disease was elevated and the more severe lipid lesions the higher the degree. Conclusion It is positively correlated between the degree of abnormal lipid profile and the extent of lower extremity vascular disease among elderly patients with T2DM. The higher degree of lower extremity vascular ather- osclerosis can affect blood circulation of lower extremities. Positive therapy about effects of lipid lowering can prevent type 2 diabetes in elderly patients with lower extremity vascular disease from occurrence and development.
