不典型动脉导管未闭的多普勒超声表现及诊断

Detection of Atypical Patent Ductus Arteriosus by Doppler Echocardiography

目的探讨不典型动脉导管未闭（ＰＤＡ）的多普勒超声心动图表现及诊断价值。方法应用二维、频谱及彩色多普勒超声心动图对３４例不典型ＰＤＡ进行探查并与手术或造影资料对照。结果所有病例切面超声均不能直接显示ＰＤＡ的解剖结构图像。频谱及彩色多普勒超声对不典型ＰＤＡ的确诊率为８２．４％，疑诊率为８．８％，漏诊率为５．９％，误诊率为２．９％。结论仔细的彩色多普勒超声心动图探查对不典型ＰＤＡ仍具有高的诊断价值。

Objective\ This study was designed to determine the ability of Doppler echocardiography for detecting atypical patent ductus arteriosus(PDA).\ Methods\ Echocardiography with 2D,spectral Doppler and color Doppler flow imaging was performed in 34 patients.The echocardiographic findings were confirmed surgically or cineangiographically.\ Results\ 2D echocardiography showed negative findings in all patients,spectral Doppler and color Doppler flow imaging detected certain but atypical PDA shunts in 28(82.4%) patients,suspected PDA shunts in 3(8.8%) patients,no PDA shunts in 2(5.9%) patients and bronchial artery shunt in 1(2.9%) patient.\ Conclusion\ With careful interrogation and avoiding the affected factors,spectral Doppler and color Doppler flow imaging remained a powerful tool for detecting PDA.
