摘要 目的探讨增殖细胞核抗原(PCNA)、p53与肾癌生物学行为的关系。方法采用免疫组化S-P法检测PCNA和p53在肾癌不同组织学类型、病理分级和临床分期的表达情况。结果(1)透明细胞癌与颗粒细胞癌间PCNA的表达差异无显著性(P>0.05),梭形细胞癌PCNA表达强阳性;(2)PCNA的表达与肾癌的病理分级及临床分期相关(P<0.05),随着肿瘤分组和分期增高PCNA表达呈上升趋势,高分期组(Ⅲ+Ⅳ期)PCNA表达明显高于低分期组(Ⅰ+Ⅱ期)(P<0.005);(3)p53阳性表达与组织学类型、临床分期无相关,与肿瘤细胞的病理分级、分化程度相关;(4)PCNA高表达者和p53阳性表达者5年生存率明显分别低于低表达者和阴性表达者(P均<0.05)。结论PCNA、p53表达与肾癌分组及术后生存期具有相关性,其阳性表达是评估肾癌预后的重要指标。 Objective\ To investigate the expression of proliferating cell nuclear antigen(PCNA) and p53 in renal carcinoma and its significance in biological behavior.\ Methods\ Immunohistochemical detection of PCNA and p53 expression was carried out in 40 cases of renal carcinoma.\ Results\ There were no significant difference in these expressions between clear cell and granular cell carcinoma while the expression rates were higher in spindle cell carcinoma,significant relationship was observed between PCNA expression and the cellular type,grade,stage of the tumor and prognosis.p53 expression was related to the grade of the tumor and prognosis.\ Conclusion\ PCNA and p53 might serve as a marker in the evaluation of the degree of malignancy and prognosis of renal carcinoma.
