基层医院门诊处方不合理用药分析

目的分析不合理用药情况及其原因,找到合理可行的解决办法。方法每月随机抽查400张左右的门诊处方进行点评分析和信息录入,然后利用数据库软件进行统计分析。结果门诊不合理用药现象包括用法用量不当、不合理选用药物、注射剂配伍禁忌、药物不良相互作用、重复用药等。出现这些不合理用药现象的原因,并非只是医师的失误,药师、护士也没有在其中起到用药审核和监督把关的作用。讨论要做到合理用药,必须建立一个医师、药师、护士共同协作的合理用药系统,同时应建立医院药学信息系统,采用电子处方和合理用药监测系统,提高医院临床用药水平和医疗服务质量。

Results The causes of irrational usage of drugs include ： the incorrect dosage of drugs, the unreasonable choice of medicine, the incompatibility of the various injections, the undesirable mutual impact of the medicine, and the inconsequent usage of the same medicine. The causes of irrational usage of drugs are not only the mistakes of the doctors, but also pharmacists and nurses had neglected their inspection duty. Conclusion In order to use the drugs rationally, we must establish a cooperative and assistant system between the doctors, pharmacists, and nurses. The hospital pharmacy information system also needs to be improved at the same time, using the electronic prescription and rational drug usage monitoring system.
