阻塞性睡眠呼吸暂停低通气患者血浆TNF-α水平的变化 被引量:1

The changes of plasma TNF-α in patients with obstructive sleep hypopnea apnea syndrome

摘要 目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血浆肿瘤坏死因子α(TNF-α)水平的变化。方法48例多导睡眠图确诊的男性OSAHS患者,按照呼吸暂停低通气指数(AHI)分为轻度OSAHS组(n=15),中度OSAHS组(n=16),重度OSAHS组(n=17)。另选16例年龄和体重指数(BMI)匹配的健康男性作为对照组。以ELISA法测定和比较各组间血浆TNF-α浓度。对OSAHS患者血浆TNF-α与睡眠期AHI和最低脉氧饱和度(SpO2)进行Pearson相关性分析。结果轻度、中度和重度OSAHS组TNF-α浓度分别为(11.49±1.32)、(13.29±1.73)和(14.74±2.72)pg/ml,均显著高于正常对照组的(9.29±1.74)pg/ml。血浆TNF-α水平随OSAHS程度加重而增高,与AHI呈显著正相关(r=0.714,P<0.01),与最低SpO2间呈显著负相关(r=-0.601,P<0.01)。结论OSAHS患者血浆TNF-α水平显著增高,且与OSAHS的严重程度相关。 Objective To investigate the changes of plasma tumor necrosis factor-α(TNF-α) in patients with obstructive sleep hypopnea apnea syndrome (OSAHS). Methods Based on apnea hypopnea index(AHI) measured by polysomnography, 48 male patients with OSAHS were divided into mild OSAHS group(L, 16 cases) , moderate OSAHS group(M, 15 cases) and severe OSAHS group(H, 17 cases). Sixteen healthy male subjects with matched age and body mass index (BMI) were selected as control group(C). Plasma TNF-α levels were measured by ELISA. Correlation of TNF-α with AHI and minimal pulse oxygen saturation(SpO2 ) was analyzed. Results Plasma TNF-α levels of group L,M and H were(11. 49±1.32) pg/ml, (13.29±1.73) pg/ml and(14.74±2.72) pg/ml, respectively, which were all higher than those in group C [ (9.29 ± 1.74) pg/ml] (P(0.01). TNF-a level was increased as the severity of OSAHS increased and positively correlated to AHI (r=0. 714, P〈0.01) but negatively correlated with miniSpO2 (r= -0. 601, P〈0.01). Conclusion TNF-α level is significantly increased in OSAHS patients, which is associated with the severity of OSAHS.
