血清脂联素监测对原发性高血压患者的意义

The Purpose of Serum Adiponectin Level in Essential Hypertension Patients

目的评价脂联素在原发性高血压(EH)发生、发展中的作用;分析血清脂联素水平与心血管危险因素之间的关系。方法①选择潍坊医学院附属医院门诊及住院的EH患者60例,以及体质量指数(BMI)、年龄、性别匹配的健康体检者60例。②用酶联免疫吸附测定(ELISA)法检测血清脂联素浓度;进行24h动态血压监测(ABPM)及常规的临床和实验室指标的检测。结果①EH患者血清脂联素水平明显低于BMI、年龄、性别匹配的正常对照者(P<0.01)。②EH患者lg(脂联素浓度)与BMI、腰臀比(WHR)、年龄、24h平均收缩压、24h平均舒张压、24h平均动脉压均呈负相关。回归分析显示,常规心血管危险因素中只有24h平均动脉压、BMI、年龄是血清脂联素水平的独立预测因子。结论①EH患者血清脂联素浓度低于正常对照,低脂联素血症与EH的发生、发展有一定关系。②EH患者的脂联素浓度与年龄、BMI、WHR、24h平均收缩压、24h平均舒张压、24h平均动脉压均呈负相关。24h平均动脉压、BMI、年龄是EH患者血清脂联素水平的独立影响因素。

Objective To explore the effect of serum adiponectin in essential hypertension （EH） patients and analysis the relationship between serum adiponectin level and cardiovascular risks. Methods （1）The study population consisted of 60 EH patients presented to affiliated hospital of Weifang medical university and 60 men matched according to body mass index（BMI）, age and sex as controls. （2）Serum levels of adiponectin were assessed with enzyme linked immunosorbent assay （ELISA）. Ambulatory blood pressure monitoring（ABPM） were recorded and other regular clinical and laboratory datas were assessed. Results （1）Female exhibited higher serum adiponectin level than male, but this difference is not significant （P〉0.05）. （2）Subjects with EH compared with controls exhibited lower lg（adiponetin） values （P〈0.01）. In the EH group, there was a negative correlation of lg（adiponectin） with BMI, waist-to- hip ratio （WHR）, age, 24-hour mean systolic blood pressure, 24-hour mean diastolic blood pressure, 24-hour mean blood pressure. Excluded weight, these relations still existed, though not so closely.Multiple regression analysis showed that 24-hour mean blood pressure, BMI, age were independent predictors of lg （adiponeetin） in the EH group.Subjects who had EH had abnormal circadian rhythm of blood pressure. Serum adiponectin between dipper EH and non- dipper EH had no significant difference （P〉0.05）. Conclusion （1）Hypoadiponectinemia has relationships with the pathogenesis and development of EH. （2） There was a negative correlation of serum adiponectin levels with age, BMI, WHR, 24-hour mean systolic blood pressure, 24-hour mean diastolic blood pressure, 24-hour mean blood pressure. And 24-hour mean blood pressure, BMI, age were independent predictors of serum adiponectin levels.
