先天性内耳发育畸形的MSCT表现

摘要 目的:探讨先天性内耳发育畸形的多层螺旋CT(MSCT)表现和特征。方法:采用GE Light Speed16排螺旋CT扫描仪,行螺旋扫描,取层厚0.625mm,Bone Plus骨算法,单侧DFOV9.6cm或7.5cm放大重建,行多平面重建(MPR)和容积再现技术(VR)重建。结果:47例89耳为先天性内耳畸形,其中Michel畸形4例5耳、耳蜗未发育1例1耳、共腔畸形4例6耳、阶间隔Ⅰ型(囊状耳蜗-前庭畸形)1例1耳、耳蜗发育不全Ⅰ型3例4耳、阶间隔Ⅱ型(Mondini畸形)14例24耳、耳蜗发育不全Ⅱ型2例4耳、前庭-半规管畸形5例9耳、前庭导水管扩大29例54耳(其中29耳伴随其他畸形)、内耳道畸形16例26耳(其中21耳伴随其他畸形)、内耳畸形伴颈静脉球高位12例18耳。结论:MSCT检查高分辨图像对先天性内耳骨迷路畸形的诊断、分型以及人工耳蜗植入术的筛查、术后效果评价、随访复查有重要的临床指导意义。是首选的检查方法。 Objective:To investigate the appearance and characteristics of multi slice CT(MSCT) in congenital inner ear development malformations. Methods :47 cases with sensorinueral hearing loss were examined by GE light speed CT16 scanner with 0.625 mm collimation,bone plus reconstruction way. The axial image of interested ear were constructed with a field of view of 9.6 cm or 7.5 cm,then 3D reconstruction were done with MPR and VR technique on workstation.Results:89 ears of 47 cases were diagnosed in congenital inner ear development malformation by MSCT scan.Congenital inner ear development malformation were classified in michel deformity in 5 ears of 4 cases、 cochlar aplasia in 1 ear of 1 case、 common cavity deformity in 6 ears of 4 cases、 incomplete partition typeⅠin 1 ear of 1 case、 hypoplastic cochlea type Ⅰ in 4 ears of 3 cases、 incomplete partition typeⅡ(Mondini deformity) in 21 ears of 14 cases、 hypoplastic cochlea typeⅡin 4 ears of 2 cases、vestibule-semicirclr canal dysplasia in 9 ears of 5 cases、 enlarged vestibular aqueduct in 54 ears of 29 cases and including with other malformation in 29 ears、 internal auditory canal deformity in 26 ears of 16 cases including with other malformation in 21 ears and with high jugular bulb in 18 ears of 12 cases. Conclusion:MSCT play important role in diagnosis and typing in congenital bony labyrinth deformation,screening and evaluating efficacy and following up in cochlea implanation. MSCT may be the first choice of examination methods on congenital inner ear development malformation.
