The mechanism of ventilator-induced lung injury

摘要 呼吸机相关性肺损伤(VILI)是高吸气峰压(PIP)、大潮气量等机械通气因素产生的机械应力作用于肺组织,激活促分裂原活化蛋白激酶、转化生长因子-β、丝/苏氨酸蛋白激酶B等信号传导通路,活化磷脂酶A2,产生并活化白细胞介素、整合素等多种促炎因子,损伤肺组织。基质金属蛋白酶和中性粒细胞弹性蛋白酶在肺损伤中有着重要作用。患者因年龄、性别、基因、病理状态等因素不同,使其对VILI的易患性不同而影响预后。
