首发精神分裂症患者执行功能损害与产科并发症的关系

The relationship of executive deficits and the obstetric complications in patients with first episode schizo- phrenia

目的探讨首发精神分裂症患者执行功能损害和产科并发症的关系。方法对67例首次发病的精神分裂症患者，75例健康人进行了产科并发症调查、威斯康星卡片分类测验（WCST）。以病例组产科并发症阳性数目的平均数为界，将精神分裂症患者分为阳性组和阴性组，比较2组之间以及与对照组之间执行功能的差异。结果阳性组的持续错误数高于阴性组[（47．14±23．69）分，（35．8±21．95）分，P〈0．05]；阳性组的完成分类数、概念化水平百分比低于对照组[（3．00±2．02）分，（4．69±1．64）分，（0．42±0．24）分，（0．59±0．20）分，P〈0．05]，总错误数、持续错误数，完成第一分类所需应答数高于对照组[（54．6±23．70）分，（34．35±21．09）分，（47．14±23．69）分，（26．49±19．05）分，（36．90±41．01）分，（22．43±20．48）分，P〈0．05]；阴性组的完成分类数、概念化水平百分比低于对照组[（3．72±2．01）分，（4．69±1．64）分，（0．51±0．23）分，（0．59±0．20）分，P〈0．05]，总错误数、持续错误数高于对照组[（44．38±22．77）分，（34．35±21．09）分，（35．87±21．95）分，（26．49±19．05）分，P〈0．05]。结论首发精神分裂症执行功能损害和产科并发症有关，产科并发症加重了精神分裂症的执行功能损害，但并不是执行功能损害的唯一原因。

Objective To explore the relationship of executive deficits and obstetric complications（OC） in patients with first episode schizophrenia. Methods 67 first onset schizophrenic patients and 75 healthy controls were examined with Wiscosin Cards Sorting Test（WCST） and the questionnaire of OC. The study group was divid- ed into two groups（ OC ＋ and OC - ） based on the average number of OC incidence according to the questionary, and the executive deficits were compared to each other and to the control group separately. Results The OC ＋ group did worse than the OC - group on the perserverative errors（PE） （47.14 ± 23.69 Vs 35.87 ± 21.95, P 〈 0.05 ） ,The OC ＋ group did worse than the control group on the categories completed（CC） , the percentage of con- ceptualization level（CI% ）,the total errors（TE）, the perserverative errors（PE） and the trials of completing the first categories（TCFC） （3.00±2.02 Vs4.69±1.64,0.42±0.24 Vs0.59±0.20,54.6±23.70 Vs 34.35 ± 21.09,47.14±23.69 Vs26.49 ±19.05,36.90 ±41.01 Vs22.43 ±20.48, P〈0.05）. The OC- group did worse than the control group on the CC, the CI% ,the TE and the PE（3.72±2.01 Vs 4.69± 1.64,0.51 ±0.23 Vs0.59 ± 0. 20, 44. 38 ± 22. 77 Vs 34. 35 ± 21.09,35. 87 ± 21. 95 Vs 26. 49± 19. 05, P 〈 0. 05）. Conclusion The executive deficits of first episode schizophrenia are significantly related to OC,and OC may ag- gravate the executive deficits but may not be the exclusive reason.
