40例主动脉夹层早期诊断分析

Early diagnostic analysis for 40 cases of aortic dissection patients

目的:探讨主动脉夹层(AD)的早期诊断要点。方法:分析总结我科40例疑诊AD患者的临床资料,40例全部或部分行心电图、X线胸片、彩色多普勒超声心动图,CTA。结果:上述检查中,诊断符合率分别为0%(0/40)、25.0%(3/12)、88.9%(32/36)、100%(13/13)。结论:心电图、X线胸片对AD不能明确诊断,仅可做筛选;超声心动图对AD诊断符合率较高可作首选;CTA对AD起确诊作用,根据临床症状和体征,也能初步诊断AD。

Objective： To discuss the early diagnostic features of aortic dissection. Methods： Forty aortic dissection patients（all or partially） were performed electricardiogram, chest X ray, color Doppler echocardiography and CT angiography. Results： The diagnostic accuracy of above examinations were： 0% （0/40）, 25.0%（3/12）, 88.9%（32/36）, 100%（13/13） respectively. Conclusion： Electrocardiogram and chest X- ray can only be used as a selective diagnostic method. Color Doppler echocardiography has high diagnostic accuracy and therefore can be the first option for diagnosing aortic dissection. CT angiography can be used for the confirmed diagnosis. It is necessary to mention that aortic dissection can also be initially diagnosed according to the clinical symptoms and signs of the patient.
