新型股骨带锁髓内钉平台定位器的设计与临床应用

摘要 [目的]通过对股骨带锁髓内钉平台定位器进行改良,降低股骨髓内钉内固定手术难度,缩短手术时间,减少医源性损伤。[方法]该课题组于2004年改进股骨带锁髓内钉平台定位器的设计,2008年申请并获得国家实用新型专利。2005年1月-2008年12月间用于治疗股骨干骨折31例,平均随访34个月。[结果]术中应用获得专利的新型平台定位器,所有31例手术都顺利完成,远端锁钉均一次成功安装,手术时间为45~60min,平均为49min。随访16~48个月,平均随访34个月。术后患者均达到临床愈合,骨折愈合时间最短4个月,最长7个月,平均6个月。未发生锁钉困难、锁钉失败、断钉等并发症。[结论]通过技术和装置的改良可以获得较高的远端锁钉锁定成功率,使手术操作简便,缩短手术时间,减少开发症,更适用于在野战环境、应急抢险救灾行动和基层医院中推广应用。 [ Objective]To improve the design of interlocking intramedullary nail platform locator,shorten operation time and decrease operation difficulty and operation-related injury. [ Method] The new interlocking intramedullary nail platform locator was designed in 2004 and obtained a China national practical new-type patent in 2008. The new platform locator was used in 31 cases of femoral fractures from January 2005 to December 2008. The patients were followed up for an average of 34 months. [ Result] With the use of this new patent interlocking intramedullary nail plaffonn locator,31 operations were performed successfully. All distal locking nails were fixed at one time. The operative duration was 45-60 minutes, averaged 49 minutes. All the cases achieved clinical healing 'after surgery. The bone healing time averaged 6 months (4 to 7 months). All the patients were fbllowed up for 16 -48 months, averaged 34 months. No complications occurred, such as locking disabilities and broken nails. [ Conclusion] The improvement of the technology and equipment can enhance distal nail locking success rate, shorten operation time, decrease operation difficulty and operation-related injury. This interlocking intrameduilary nail platform tocator is more suitable for field environment, emergency rescue, disaster relief and primary hospital.
