距骨开放性骨折中污染离体骨折块的处理策略

摘要
距骨骨折发生率较低，在所有骨折中的发生率为0．32％^[1]，且多为高能量损伤所致，20％～25％为开放性骨折^[2]，多伴有严重的软组织挫裂伤，而且距骨本身无单独的血供，故骨折后易发生骨折延迟愈合或不愈合、距骨缺血性坏死、感染等并发症。距骨开放性骨折常常有骨折碎块游离脱出于体外而受到污染，对于这些既失去血供又被污染的离体骨折块，是否应该丢弃以避免骨髓炎的发生？
