应用长庚法修复单侧唇裂22例

Clinical application of Changgeng's technique on repairing unilateral cleft lip(22 cases attached)

目的探讨应用长庚法修复单侧唇裂的整复效果。方法应用长庚法修复22例单侧唇裂。按长庚法设计切口,应用健侧唇上端形成一组织瓣(c瓣)旋转插入健侧下降后形成的鼻小柱下方的缺损内;将口轮匝肌解剖复位,再造人中嵴;游离并悬吊固定患侧鼻翼软骨。结果22例中创口均Ⅰ期愈合。除2例外,患侧唇的高度、长度与健侧一致,患侧鼻底宽度适中且有较好的丰满度,患侧人中嵴较明显,红唇部形态自然、丰满。结论应用长庚法修复单侧唇裂,可基本恢复患儿患侧唇高,唇长及鼻孔宽度,手术整复效果满意;但Ⅲ度唇裂患侧唇峰下降不足。

Objective To evaluate clinical effects of unilateral cleft lip repairing by using Changgeng＇ s methods. Methods 22 cases were treated with Changgeng＇ s methods. The triangle skin flap of the upper lip was used to insert the coloboma un- der columella nasi. Orbictdaris oris muscle was reposited in one stage and phihrum columnreconstructed. Lifting and fixing ala nasi cartidge was conducted. Results The wounds of all cases were healed in one stage. The height and width of affected side were symmetric except 2 cases. The ridge of philtrum was protrudent and the red lip~ form was natural and abundant. Conclu- sion Changgeng＇s method is an ideal method for repairing unilateral cleft lip.
