带蒂胸脐皮瓣修复前臂及手部电击伤及热压伤伤面的疗效

Clinical Effects of the Pedicled Thoracoumbilicus Flap in repairing Wound of Electrical Wounds and Heat Crushes on Forearm and Hand

目的探讨胸脐皮瓣修复前臂及手部电击伤创面的疗效。方法对8例手和前臂电击伤热压伤创面行带蒂胸脐皮瓣修复,皮瓣面积从14 cm×8 cm～25 cm×12 cm,观察皮瓣的成活情况、色泽、弹性,有无座痕挛缩导致的功能障碍及供瓣区愈合情况。结果8例皮瓣完全成活,术后18 d断蒂,术后随访12～26个月,所有患者均对前臂、手部功能和外形表示满意,但供区瘢痕较明显。结论带蒂胸脐皮瓣修复前臂及手部电击伤及热压伤创面具有操作简单、供区隐蔽、风险小、疗效好的优点。

[ Objective] To investigate the clinical effects of the pedicled thoracoumbilicus flap for repairing the wound of electrical wounds and heat crushes on forearm and hand. [Methods] 8 cases suffering from electricity wounds and heat crushes on forearm and hand were treated with the pedicled thoracoumbilicus flap. The area of flap ranged from 19cm × 8 cm to 25 cm × 12 cm. Flap survival, the color, flexibility, dysfunction caused by block scar contracture and healing status of flap donor site were observed. [ Results] 8 cases＇flaps survived completely. The pedicle was broken 18 days after the operation; following-up from 12 to 26 months showed satisfied appearance and function of forearm and hand, but scar in flap donor site was obvious. [ Conclusion ] The pedicled thoracoumbilicus flap can be applied to repair wound of electrical wounds and heat crushes of forearm and hand, for its advantage of lower risk, hided donor site and better effects.
