心肺功能运动试验和静态肺功能对评判COPD患者病情轻重程度的比较

摘要 【目的】比较心肺功能运动试验(CPET)和静态肺功能(PFT)对评判慢性阻塞性肺疾病(COPD)患者病情轻重的相关性。【方法】98名处于稳定期的COPD患者分别进行PFT和CPET检查;以PFT中FEV,%(占预计值%)和CPET中最大公斤摄氧量(VO2max/kg)两种不同方法判断COPD患者病情轻重并作相关性分析,同时以VO2max/kg判断COPD病情重于以FEV1%判断的患者与余下的患者作比较。【结果】两者方法判断COPD患者病情轻重无相关关系;以VO2max/kg判断病情较重的患者其年龄、FEV1%、V02max/kg、无氧阈占最大摄氧量比值(AT/VO2max)、最大呼吸储备(BRmax)与其他患者比较差异均有显著性。【结论】①CPET和PFT判断COPD患者病情并无相关性,但CPET反映病情更全面。②CPET有助于发现COPD患者潜在的心功能问题。 [Objective] To compare the correlation between cardiopulmonary exercise tests (CPET) and the rest- ing pulmonary test in the classification of chronic obstructive pulmonary disease(COPD) patients according the severity. [Methods] A total of 98 COPD patients in the stable stage were given CPET and PFT. All the patients were classified according to the FEV1% and the maximal oxygen uptake per kilogram of body weight (VO2max/kg) respec- tively and the correlation between the two classifications were studied. In the patients more impaired according to the VO2max/kg, some parameters were compared with the remaining patients. [Results] There was no significant correla- tion between the two classification methods. There was significant difference in age, FEV1 %, VO2max/kg, anaerobic threshold (AT) /VOz maximal breathing reserve (BRmax) between the patients more impaired according to VO2max/kg and the remaining patients. [Conclusion]There is no significant correlation between the classification ac- cording to CPET and the classification according to PFT, although CPET is believed to be more accurate. CPET is helpful to find the potential heart failure in COPD patients.
