摘要
内窥镜在神经外科的应用虽然有近百年的历史,但是直至今天,在现代光学技术、影像技术及显微手术器械高度发展的情况下,才真正发挥出优势。现在,作为外科手术的一个重要技巧,神经内窥镜技术已从诊断和治疗两方面渗入到神经外科的各个领域。其适应证包括:脑积水、各种颅内囊性病变、脑深部肿瘤、鞍区病变、脊髓空洞症、椎间盘脱出以及交感神经切除等,指征已较过去明显扩大。神经内窥镜手术的优点是:创伤小、安全、康复快、费用低,代表了神经外科发展的趋势。缺点是术野小、应用范围有限。但可以预期,只要在实践中不断与影像神经导航技术及开颅显微手术结合,内窥镜神经外科必将为21世纪神经外科作出新的贡献。
Although endoscopy has heen ap-plied in neurosurgery for nearly one century, it hasn't shownits great advantages until contemporary techniques in optic,image and microsurgical instruments been developed dramat-ically quite recently. Nowadays, neuroendoscopy plays animportant role in both diagnosis and treatment in the field ofneurosurgery. The endoneurosurgical indications include:hydrocephalus, intracranial cystic lesions, brain tumors indepth, lesion at sella region, CPA disorders, syringomyelia,disks protrution and sympathectomy, etc. As far as the pre-sent, the indications for endoneurosurgery have beenextending obviously. The strong points of endoneu-roscopy are: minimally invasive, safety, quick recoveryand lower cost, which also represent a tendency to neu-rosurgery So long as the neuroendoscopic techniquegets further advance with the combining with imageguided neuronavigation and open microsurgery, the en-doneurosurgery expectably, will demonstrate a newcontribution to neurosurgery in 21st century.
