胰腺癌的CT诊断及其与B超的对比研究

CT Diagnosis of Pancreatic Cancer and the Comparative Study with B-US

目的:研究胰腺癌胰周组织侵犯征象,提高 CT 术前分期的诊断准确率。并为诊断胰腺癌提供合理选用 CT、B-US 的依据。材料与方法:总结64例经手术、病理证实的胰腺癌,对术前 CT、B-US 的诊断准确率及各种征象检出率进行对比,并进行回顾性分析。结果:CT、B-US 的诊断准确率,直接及间接征象、部分胰周侵犯、肝转移征象的检出率相似。CT 在胆总管阻塞端,胰周大血管侵犯,腹膜后淋巴结转移的检出率优于 B-US。结论:CT 对胰腺癌的术前分期有优势。单发胰周大血管Ⅲ型侵犯(无脏器及淋巴转移)有可能切除肿瘤,Ⅳ型及Ⅳ型以上侵犯均未能切除肿瘤。

Purpose:The aim of this study was to improve the diagnostic accuracy of CT for the preoperative stage of the pancreatic carci- noma by means of studing tumors invasive sign of peripancreatic structures,to provide reference basis for choosing CT or B-US to examine the pancreatic cancer.Materials and methods:Sixty-four cases of pancreatic cancer proven by operation and pathology were reported.The preoperative diagnostic accuracy and detecting rate of all sings were compared between CT and B-US.All cases were analysed retrospectiv- ely.Results:B-US was similar to CT in diagnostic accuracy of pancreatic cancer,in detecting rate of the direct and indirect signs,in parts of invasive signs and the sings of metastatic tumor in the liver(P>0.05).CT was superior to B-US in the detecting rate of obstructive tip in the bile duct,of earcinous invasion to peripancreatic blood vessels and of lymphs of retroperitoneum.Conclution:CT has superiority in the stage of pancreatic carcinoma.In type Ⅲ involvement of single blood vessel,which has no metastasis of lymph and organ,the resection of tumor was possible.No pationts with type Ⅳ.Ⅴ.Ⅵ Ⅴascular involvement had tumor removed.
