住院药历的格式与书写要求探讨

Discussion on Writing Form and Request of Inpatient Medication Records

目的探索一种规范、方便、实用的药历格式。方法通过临床实践及参考文献,建立了适合本院临床药学服务的药历格式并对药历书写做出具体要求。结果与结论建立规范化的药历对临床药师的工作实践具有重要意义。

Objective To explore a kand of standard, convenient and suitable writing form of medication record. Methods Based on clinical practice and references, the inpatient medication records that suited to the clinical pharmaceutical care in our hospital and concrete request of writing have been established. Results and Conclusions It is significant for working of the clinical pharmacist to set up standardized medication record.
