三维DCE MRA在门静脉和肝脏静脉系统的应用 被引量:27

Applications of 3D DCE MRA in the imaging of the portal and hepatic venous system

摘要 目的探讨门静脉和肝脏静脉系统三维动态增强磁共振血分成像(3DDCEMRM)检查的最佳技术,评价3DDCEMRA在该系统中的使用价值。方法门静脉和肝脏静脉系统行3DDCEMRA108次,二维(2D)DCEMRA10次。比较两种力法显示门静脉右支的能力,不同层厚对3DDCEMRA显示门静脉右支的影响,并对门静脉、肝静脉和下腔静脉在3DDCEMRA的显示情况以及各种病变的3DDCEMRA表现一分析。结果3DDCEMRA显示门静脉右支级数优于2DDCEMRA(P<0.05),且层厚越薄,显示分支越细(p<0.01)。3DDCEMRA能较好地显示门脉主干(显示率93%)、左右分支(88%.97%)、肝静脉(81%)和腔静脉(83%),并能显示多种疾病。结论3DDCEMRA能较好地同时显示门静脉、肝静脉和下腔静脉,在门静脉和肝脏静脉系统多种疾病的显示和诊断方面具有较高使用价值。 To investigate the optimal techniques of dynamec contrast-enhaned(DCE) MRA and evaluate the diagnostic value of 3D DCE MRA in the examination of the portal and hepatic venous system.Methods 108 3D DCE MRA and 10 2D DCE MRA were performed and compared in visulization of the right Portal vein branches. The display of the right Potral vein branches was also compared between groups with different slice thickness in 3D DCE MRA exwinations. The demonstration of the portal and hepatic venous system on 3D DCE Ma and the MRA findingS in a variety of diseases were analyzed. ResultS 3D DCE MRA was better than 2D DCE MRA in visulization of the right portal vein branches(P 〈 0.05 ). 3D technique with thinner slices further improved its ability of detecting smaller vessels(P 〈 0.01 ). The visulization rates for the portal vein trunk, left and right Porbal vein, hepatic vein and inferior vena cava were 93%, 88% and 97%, 81% and 83% respectively. It can be used in a spectrum of abnormalities. Conclusion In most cases, 3D DCE MRA can clearly demonstrate the portal vein, hepatic veins, and inferior vena cava simultaneuosly. It is a valuable imaging method to detect anddiagnose various pathologic conditions involving these venous systems.
