翼状胬肉撕离联合不同角膜缘干细胞移植

Analysis of pterygium excision by tear combined different kinds of corneal limbal stem cell transplantations

目的比较翼状胬肉撕离联合自体游离结膜瓣角膜缘十细胞移植及带蒂结膜瓣角膜缘干细胞移植治疗翼状胬肉的临床疗效。方法91例（97眼）翼状胬肉随机分为A、B两组．A组45例（47眼）行翼状胬肉撕离联合白体游离结膜瓣角膜缘干细胞移植术；B组46例（50眼）行翼状胬肉撕离联合带蒂结膜瓣角膜缘干细胞移植术。术后随访6～30月，比较两组术后角膜缘干细胞植片存活及翼状胬肉复发情况。结果A组5眼移植片脱落，B组植片无脱落，（X2=5．31，P〈0．05）。A组6眼翼状胬肉复发，复发率为12．76％；B组巾1眼复发，复发率仅为2．00％；（X2=5．20，P〈0．05）。结论翼状胬肉撕离联合带蒂角膜缘干细胞移植术，植片存活率高，翼状胬肉复发率低。

Objective To observe the therapeutic effects of free conjunctival flap corneal limhal stem cell autograft transplantation and pediele conjunetival flap corneal ]imbal stem cell trans-plantation after pterygium excision by tear. Methods 91 patients （97 eyes） were randomly divided into two groups： free conjunctival flap corneal limbal stem cell autograft transplantation （group A, 47 eyes） and pedicle conjunctival flap corneal limbal stem cell transplantation （group B, 50 eyes）. All patients had been fllowed up ror 6 - 30 mouths, then compared the survival rate of the grafts and the recurrence rate. Reasults 5 of 47 grafts ablated in group A and no graft dropped in group B. The difference is significant between two groups （X2 =5.31 ,P 〈0. 05）. In grnup A, 6 of 47 eyes were recurred. The recurrence rate was 12.76%. In group B, 1 of 50 eyes were recurred. The recurrence rate was only 2.00%. The difference is significant between two groups （X2 = 5.20, P 〈 0.05 ）. Conclusion Pterygium excision by tear combined pedicle conjunctival flap corneal limchal stem cell transplantation can make the survival rate of the grafis higher and the recurrence rate lower.
