冠心病围术期心肌肌钙蛋白I动态变化及意义 被引量:1

摘要 【目的】观察心肌肌钙蛋白I(cTnI)与肌酸激酶同功酶(CK-MB)在冠状动脉旁路移植术(CABG)围术期变化,比较二者对心肌损伤的诊断差异。【方法】连续152例病人在心脏停跳体外循环(CPB)下行CABG术,平均年龄62(S=21)岁,范围39~80岁,平均每例搭桥2.7(1~5)支,平均CPB时间113(S=54)min,平均主动脉阻断时间56(S=31)min。围术期9个时间点取静脉血标本,测定cTnI与CK-MB,结果用"均数±标准差"表示。【结果】cTnI与CK-MB均于主动阻断钳开放后就明显升高,开放后10h达峰值,分别为(0.72±0.33)μg/L与(56.3±28.6)IU/L。CK-MB术后5d恢复至术前水平,而cTnI术后10d才恢复至术前水平。13例围术期心肌梗死(PMI)患者术后各时间点cTnI均显著高于无PMI者,术后10h达峰值,为(3.96±1.73)μg/L,且术后10d仍明显高于正常范围,为(1.23±0.65)μg/L;13例中有7例术后因PMI而死亡。而无PMI者,无一例因心脏原因死亡。【结论】cTnI与CK-MB在CABG术后前期的动态变化相似,但cTnI高于正常值的时间长,对心肌缺血损伤诊断时间窗宽于CK-MB。因此cTnI诊断心肌损伤的晚期敏感性高于CK-MB。而且,围术期cTnI的检测也是预测CABG术后PMI及预后的可靠指标。
