青春期前特发性矮小儿童IGF-1、骨代谢指标水平变化及相关分析 被引量:9

Change and relative analysis of serum levels of insulin-like growth factor-1and bone metabolization indexes in idiopathic short stature children

摘要 【目的】研究特发性身材矮小(idiopathic short stature,ISS)儿童胰岛素样生长因子1(IGF-1)、骨代谢生化指标水平,以及ISS儿童IGF-1与骨代谢指标、骨龄/年龄比值(BA/CA)和身高标准差得分(HtsDs)之间的相关性。【方法】测定37例ISS儿童和30例年龄、性别匹配的正常健康儿童的血清IGF-1、骨代谢生化指标骨钙素(OC)、Ⅰ型胶原蛋白羧基末端前肽链(PICP)I、型胶原交联羧基末端肽(ICTP),比较两组水平差异;对IGF-1与骨代谢生化指标、骨龄/年龄比值(BA/CB)和身高标准差得分(HtsDs)之间进行Pearson相关分析。【结果】ISS组血清IGF-1、OC、PICPI、CTP水平显著低于正常对照组(P<0.05).ISS组IGF-1分别与OC、PICP、ICTP、BA/CA、HtsDs呈正相关(P<0.05)。【结论】ISS儿童骨转换水平明显降低,IGF-1水平降低与ISS儿童骨转换及骨形态学改变显著相关,应警惕ISS患者成年后骨质疏松症的发生,给予干预治疗。 [Objective] To mensure serum levels of insulin-like growth factor-1 and bone metabolization indexes, and to investigate the correlations between serum levels of insulin--like growth factor-1 (IGF-1) and bone metabolization indexes,BA/CA,HtsDs in the children of idiopathic short stature (ISS). [Methods] Serum IGF-1,osteocalcin(OC),carboxyterminal propeptide of type Ⅰ procollagen (PICP), type Ⅰ collagen cross-linked C-terminal telopeptide (ICTP) were mensurated in 27 cases in ISS children,and compared to 30 cases age-and sex-matched healthy controls. The relationship between IGF-1,OC, PICP, ICTP, BA/CA and H tsDs were analyzed with Pearson correlation analysis. [ Results] Compared with the normal group, The serum levels of IGF-1, OC, PICP, ICTP in ISS group were significantly decreased(P〈0.05). IGF-1 was positively correlated with OC,PICP,ICTP,BA/CA and HtsDs(P〈0.05). [Conclusions] Compared with the normal group, the level of bone turnover markers are significantly lower in ISS children. The lower serum level of insulin-like growth factor-1 might is significantly related to the changes of bone turnover markers and bone morphology. To guard against osteoporosis and ISS should be intervened.
