子宫肌瘤阴式全切除术49例临床探讨

Clinical research and analysis on vaginal hysterectomy for 49 cases of hysteromyoma

目的探讨子宫肌瘤经阴道子宫全切除术的可行性、适应证、禁忌证、优越性及临床应用价值,并进行效果分析。方法对49例有子宫切除手术适应证者行经阴道子宫切除术,取同期经腹切除子宫49例作对比分析。结果阴式组较腹式组手术时间长,术中出血稍多(P<0.05),而术后肛门排气时间、使用抗生素天数、术后镇痛例数、体温、术后住院日阴式组少于腹式组(P<0.01),平均费用、术后患者满足度阴式组明显高于腹式组(P<0.01)。结论阴式全子宫切除术具有术后疼痛轻、恢复快、住院日缩短,患者满意度高等优点,是一种安全、手术效果好、副反应少的手术方式。

Objective To study the feasibility,indications,contraindications,advantages and clinical application value of vaginal hysterectomy for hysteromyomas.Methods 49 cases were perfomed Vaginal hysterectomy,at the same time 49 cases were performed abdominal hysterectomy as control group.Results The operation time and blood loss in Vaginal group were more than that in abdominal group（P〈0.05）.Passage of gas by anus time,the use of antibiotics,relieving pain,body temperature and the hospital stay in vaginal grou pwere less than in abdominal group （P〈0.01）.The average cost and patient＇s satisfaction of vaginal group was significantly higher than that in abdominal group （P〈 0.01 ）.Conclusion Vaginal hysterectomy with lighter pain, quicker recovery, shorter hospital stay and higher patient satisfaction, is a safe, effective and less side effects surgery.
