腰椎小关节骨关节炎与椎间高度及年龄的相关性研究 被引量:2

The Relationship of Facet Joint Osteoarthritis to the Heights of Intervertebral Disc and Age in Lumbar Spine

摘要 目的探讨腰椎小关节骨关节炎和椎间盘高度、年龄的相关性.方法收集120例腰椎间盘退患者下腰段(L3-L5)的腰椎CT和MRI影像学资料,对腰椎小关节骨骨关节炎、椎间高度降低程度进行分级,采用SSPS17.0统计软件包,采用有序Logistic回归分析(Ordinal Regression)腰椎小关节骨骨关节炎和椎间高度降低、年龄的相关性.结果(1)年龄因素:年龄与腰椎小关节骨关节炎呈正相关(P<0.001),有统计学意义.(2)椎体间高度降低因素:相对于椎体间高度正常者(GL=5),只有Ⅱ度椎体间高度降低者(GL=3)的腰椎小关节骨关节炎病情程度明显加重(P=0.037),有统计学意义.而Ⅰ度、Ⅲ度和Ⅳ度椎体间高度降低者(GL=4、GL=2、GL=1)的腰椎小关节骨关节炎病情程度无明显加重,与正常者基本一致(P≥0.183),无统计学意义.结论自然老化是腰椎小关节骨关节炎主要因素,多数腰椎椎小关节骨关节炎与椎间盘高度减低的相关性无统计学意义. Objective To evaluate the relationship between facet joint osteoarthritis and the heights of intervertebral disc and age in lumbar spine. Method We collected comuputed tomographyc (CT) and magnetic resonance image (MRI) data of 120 cases of patients with lumar disc degeneration. We classified facet joint osteoarthritis into four grades, the heights loss of disks into five grades and age into five groups, All the data were analyzed by Logistic Ordinal Regression ways of SPSS 17.0 statistics for the correlations. Results (1) Lumbar facet joint osteoarthritis had significant positive correlation with age (P 〈 0.05 ). (2) Lumbar facet joint osteoarthritis had significant correlation with the heights loss of disks grade 2 (P 〈 0.05) , had not significant correlation with the heights loss of disks grade 1, grade 3 and grade 4 (P 〉 0.05 ). Conclusions Natural aging is basic reason for Lumbar facet joint osteoarthritis. The majority of lumbar facet joint osteoarthritis has not significant correlation with the heights loss of disks.
