小切口手法碎核治疗硬核白内障800例临床分析

Clinical analysis of 800 cases of small incision cataract extraction by the hard nucleus breaking technique

目的:探讨小切口手法碎核对硬核性白内障摘除人工晶体植入术的临床疗效。方法:作反眉弓5.5 mm 180°巩膜隧道切口,采用手法碎核技术对800例(800眼)晶状体硬核白内障摘除并植入折叠式或PMMA人工晶体。结果:800例(800眼)中,792眼成功完成手法碎核,未成功8例。术后1周,裸眼或球镜矫正视力≥0.5者占92%,术后1周平均散光为(1.22±0.75)D。结论:反眉弓巩膜隧道小切口手法碎核人工晶体植入术治疗硬核性白内障简便易行,不需特殊设备,其效果可与超声乳化术媲美,可有效地减少术后角膜散光,早期获得良好的视力,降低手术费用。

Objective： To discuss the clinical effect of small incision cataract extraction by manual nucleus division and intraocular lens （IOL） implantation. Methods：A scleral tunnal incision of 5.5 mm, 180° and anti-geisoma was mad, and the cataract extraction of hard nucleus was performed by manual nucleus division,then foldable or PMMA intraocular lens was implanted.Results： Of the 800 cases （800 eyes）, 792 cases was performed successfully, 8 cases failed. 92% of eyes, naked or corrected, attained a visual acuity of 0.5 or better in one week. The average postoperative astigmatism was （1.22±0.75） D in one week. Conclusion：The small incision cataract extraction by manual nucleus division and intraocular lens （IOL） implantation is an simple operation, which can reduce the postoperative astigmatism effectively and enable the patients to get a good visual acuity. And no special appliance is required. But the effect is not bad than the phacoemulsification, moreover, the expense is low.
