摘要 目的探讨微波消融犬肺组织的可行性,为临床应用提供实验依据。方法以微波功率20 W、40 W、60 W及辐射时间600 s的不同参数组合作用于15只家犬肺组织,测量微波消融灶的大小,术后1 h内取肺组织送病理学检查。结果微波消融术中1只家犬因心脏损伤死亡,余均对微波消融耐受良好。不同功率消融范围不同,40 W消融范围最大,20 W次之,60 W消融灶最小。消融灶的形态椭圆形,近球形。消融灶病理呈特征性表现,从内到外为中心碳化区、中间凝固区及周边反应区。结论微波消融肺组织创伤小,安全可行;40 W能形成(3.12±0.08)cm2的消融面积,适合于活体犬肺组织的微波输出功率。 Objective To evaluate the feasibility of microwave ablation(MA) for canine lung tissue,and to provide experimental evidences for clinical application.Methods Pulmonary tissues of 15 domesticated canine were ablated with different microwave output power(20 W,40 W,60 W).Ablation lesions were excised within 1 h after MA and pathological examine was performed.Results ①One canine died during MA because of heart injury,the others tolerated the procedure well.②The areas of ablation was different according to different power.The ablative area of 40 W was the largest,the mean diameter was(1.72±0.13) cm,the mean area was(3.12±0.08) cm2,while the ablative area of 20 W was the second large,with the mean diameter of(1.07±0.05) cm,the mean area of(2.24±0.31) cm2,and the ablative area of 60 W was the smallest([1.14±0.07] cm2),with diameter of(1.19±0.23) cm.The shape of the ablative lesion was ellipse near global.③From the center to peripheral area,the pathologic changes were characterized as carbonization,coagulation necrosis,and peripheral reactive band.Conclusion Microwave ablation for canine lung tissue is minimally invasive and feasible,and 40 W is suitable output power producing ablative area of(3.12±0.08) cm2 in the intravital canine lung tissue.
