咽淋巴环非霍奇金淋巴瘤的CT表现

CT Diagnosis of Non-Hodgkin's Lymphoma in the Waldeyer's Ring

目的:探讨咽淋巴环非霍奇金淋巴瘤的CT表现,以提高诊断及鉴别诊断的准确性。方法:回顾性分析19例临床资料完整,原发于咽淋巴环淋巴瘤的CT表现。结果:B细胞来源占68.4%(13/19),其中弥漫性大B细胞淋巴瘤有11例。T细胞来源占31.6%(6/19)。6例腭扁桃体淋巴瘤表现为扁桃体窝内边界清楚的类圆形软组织肿块,向咽腔内突出生长。6例鼻咽淋巴瘤和7例咽侧壁淋巴瘤均表现为咽腔内弥漫生长的软组织肿块,病变范围较大,无颅底和周围结构侵犯。淋巴瘤密度均匀,呈轻中度强化,无坏死和囊变,肿块与周围界限清楚,19例淋巴瘤中有12例颈部淋巴结肿大。结论:CT能清楚显示咽淋巴环淋巴瘤的位置、形态及范围,有无颈部淋巴结肿大和周围组织的侵犯,对临床诊断和治疗有重要参考价值。

Objective： To investigate the CT appearenees of non-Hodgkin＇s lymphoma（NHL）in the Waldeyer＇s ring, and to improve the accuracy of diagnosis and differential diagnosis. Methods： CT findings of 19 cases with NHL in the Waldeyer＇s ring were retropectively analyzed. Results： Of 19 patients, 13 cases originated from B-cell, and 6 cases from T-cell, but the diffuse large B-cell lymphoma occurred in 11 of 13 patients. The round soft tissue mass with protruded into pharynx was displayed in 6 cases of lymphoma of the tonsil. The diffuse soft tissue mass without skull base and surrounding tissues invasion was presented in 6 cases of lymphoma of nasopharynx and 7 cases of lateral pharynx. The edge of the masses was clear, the density was syrnmatric, without necrosis and cyst and slight to moderate enhancement. The cervical lymph node enlargement was occurred in 12 of 19 patients. Conclusion： CT has important value for clinical diagnesis and therapy of lymphoma, showing clearly the site, shape and extension of NHL in the Waldeyer＇s ring, with or without the cervical lymph node enlargement and surrounding tissue invasion.
