颈动脉粥样硬化斑块的磁共振成像特点 被引量:2

摘要 影像学检查在缺血性卒中的诊治、预后评估中的价值毋庸置疑。随着以"病人为中心"、在循证医学基础之上的个体化医疗观念的不断深入,临床对影像学检查的要求也越来越高。在急性缺血性卒中的影像学检查中,准确、快捷是首要的;而对非急性缺血性卒中而言,准确和全面则是下一步治疗和预防的基础。本期的几篇研究论文对此进行了初步的尝试。基于磁共振成像(magnetic resonance imaging,MRI)表观弥散系数(apparent diffusion coefficient,ADC)对急性缺血性卒中梗死核心以及缺血半暗带的研究,摒弃了传统的MRI灌注加权像(perfusion-weighted imaging,PWI)和弥散加权像(diffusion-weighted imaging,DWI)不匹配模型的作法,省却了PWI的检查,仅仅对DWI和ADC的数据进行分析,获取梗死核心和缺血半暗带,大大简化了急诊状态下的MRI程序,缩短了检查时间,显示出广阔的临床应用前景。基于磁共振血管成像(magnetic resonance angiography,MRA)的颈动脉分叉血流动力学研究,从计算机模拟上升到在体测量,使得靶血管局部血流动力学的研究更贴近临床,为临床进一步认识动脉粥样硬化斑块的转归建立了切实可行的影像技术平台。毛细血管表面通透性的影像学研究为临床预测缺血性卒中静脉溶栓治疗后的出血转化提供了一个切实可行的技术方法。可以说,临床与影像的紧密结合使得影像学研究更具生命力。
