2007年8月～2008年11月我院处方质量分析

Quality of Prescriptions Prescribed from Aug.2007 to Nov.2008 in Our Hospital

目的:了解新的《处方管理办法》实施以来我院的贯彻执行情况,提高处方质量,保障医疗安全。通过对我院西医门诊处方用药的合理性进行分析,确保患者用药安全、有效、经济、合理,为今后医院临床合理用药提供参考。方法:回顾性地抽查本院2007年8月～2008年11月的门诊处方共计1 880份,逐一进行审核及统计分析。结果:不合格处方577张,占所查处方的30.69%。其中未写诊断结果、超剂量用药和用法含糊不清是处方质量管理中存在的主要问题。结论:我院门诊用药基本合理但仍存在一定问题,处方平均金额呈下降趋势,不合理处方中存在的问题应引起医师的注意,同时临床医师与药师需共同加强药学知识的学习。

OBJECTIVE： To probe into the implementation of the new Prescription Management Method in our hospital in order to improve the prescription quality and guarantee medical security,and to ensure patients safe,economical,rational and effective medication for clinical reference of rational drug use through analysis on the rationality of the outpatient prescriptions of western medicine.METHODS： A total of 1 880 outpatient prescriptions prescribed between Aug.2007 and Nov.2008 in our hospital sampled retrospectively were checked and analyzed statistically. RESULTS： Of the 1 880prescriptions reviewed, 577 （30.69 % ）were found to be unqualified. The main problems appeared m the quahty management of prescriptions were lack of diagnosis, overdose and ambiguous usage. CONCLUSION ： The outpatient drug utilization in our hospital was rational on the whole yet it far from perfect. The average amount of money for each prescription assumed a downward trend. Clinicians should attach great importance to the problems likely occur in irrational prescriptions. Meanwhile, both clinicians and pharmacists should strengthen their learning on pharmacy.
